Mel Presland and Tammy Koay are part of the multidisciplinary team at Sobell House and are involved with the inpatient unit, the community palliative care team and the hospital palliative care team as well as the day hospice.

Much of palliative and end of life care focusses on the symptoms of patients and the medicine choices needed to make the patient more comfortable and allow them to be supported in their preferred place of care. The medicines management of  patients can be complex and specialist knowledge is needed to ensure that the medication is appropriate for each patient, taking into account co-morbidities, patient preference and potentially limited routes of administration.

The pharmacist provides counselling and support for both patients and those important to them  to make best use of their medication once discharged home, with  communication to the primary care team and community pharmacy of any complex pharmaceutical issues.
