The Hospital Palliative Care Team (HPCT) supports the care and management of patients with palliative care and end of life needs in the Oxford University Hospitals Trust hospitals: the John Radcliffe, the Churchill and the NOC. Patients at the Horton Hospital are seen by the Horton’s own HPCT, based at Katherine House Hospice.
The HPCT is multidisciplinary and consists of Advanced and Specialist Nurse Practitioners, Consultants and a Specialist Registrar in Palliative Medicine, a Specialist Pharmacist and a Chaplain.
The Team supports palliative care and end of life care in the acute hospitals in two ways:
- Direct involvement with the care and management of individual patients who have been referred to the HPCT
- Facilitative and educational work aimed at raising awareness of palliative care and end of life care issues, and enabling other health care professionals to deliver appropriate palliative and end of life care
The HPCT aims to support teams in the acute setting with the management of patients with complex physical, emotional, spiritual or social needs, who are not responding to current management strategies and who meet our referral criteria. Assessment, advice and support may be concerned with management of pain and other symptoms, psychological and emotional needs, decision making about clinical interventions and options for planning care in the future.
The HPCT discusses all advice given with members of the referring team. The HPCT does not take over the care of patients with palliative and end of life care needs, but works alongside the team leading on the patient’s care. Responsibility remains with the referring team, and the HPCT seeks to avoid replicating supportive work being done by other health care professionals.
The HPCT makes information and educational resources available for other professionals via a site on the OUH intranet which includes symptom management guidelines, end of life care information, syringe driver guidelines, and referral forms. The HPCT also offers educational sessions and study days to all staff groups.
Referrals for assessment and support for individual patients who are inpatients in one of the acute hospitals should be made directly to the on-site hospital HPCT offices by telephone or by fax. Advice is always available out of hours, including weekends and Bank Holidays via telephone.

HPCT members may be contacted on their mobile telephones through the switchboard in very urgent/emergency situations, for all other referrals please use the office numbers and leave a message on the answerphone if needed – the HPCT will always respond to messages as promptly as possible.
