The Palliative Day Service offers a 12 week individualised programme for people with progressive life-limiting condition and who are being referred to Sobell House. This programme runs between 10 am and 3 pm on Mondays through to Thursdays. Following referral, patients are contacted by phone and invited to come for an introductory meeting, accompanied by their family/carer if they wish. After that, formal assessments, goal setting and reviews take place in weeks 1, 5 and 10 and completion at the end of week 12.
Depending on the person’s needs, interventions offered may include physical and psychological symptom management, physical therapies, music or art therapy, creative or complementary therapy sessions and other support services such as assisted bathing.
