We undertake work on behalf of Social & Community Services, and also the Continuing Care Service in Oxfordshire. This means that we can assess patients on the ward, and complete the relevant paperwork to obtain practical support. In some cases funding for these services can also be arranged.
Our social work team provides support to patients who wish to return home after a period in hospital, and who may require some assistance in order to remain at home. On some occasions we can assist in arranging live-in carers to support you to live in your home. Sometimes patients may not be able to go back home because it would be impractical and potentially unsafe, and social workers can assist with arranging a placement in a nursing or care home.
Once care is set up, or a nursing or care home is arranged, our social workers are also able to offer support once you have left the ward for a limited period of time. This enables us to review discharge plans, and liaise with relevant organisations for you where this is appropriate.
