The Community Specialist Palliative Care Team work alongside GPs to support the well-being of patients who have a life-limiting illness.

Support at home by our community team

Our team includes specialist nurses and doctors, as well as a physiotherapist, occupational therapist, pharmacist and benefits advisor. All members of the team have specialist training and experience in caring for people with a life-limiting illness and focus on quality of life and what it means for each person.

We see people at different stages of their illness, ranging from those who have recently been diagnosed, to those nearing the end of their life. Many people with a life-limiting illness do not need our involvement, as GPs and district nurses are able to provide the care and support that is needed.

People can be referred to our service when their needs are more difficult to manage; referral can be made by their GP, district nurse, consultant, or another health care professional. Depending on individual need, we either see people in an outpatient clinic or visit people in their home.
